How do I refer a patient to CODG or The Orthotic Centre?
Patients can be referred directly to one of our locations with a fax referral or prescription to (905) 828-1091(Mississauga), (905) 813-4539 (The Credit Valley Hospital) or (905) 540-6546 (St. Joseph's HEalthcare, Hamilton). Patients can be given a regular prescription with a diagnosis and recommendations. Please contact us if you require prescription pads or further information. What services are provided?
Orthopaedic shoes - Our experienced footwear consultants will advise your patients of the most appropriate shoes and provide expert guidance in fitting shoes. More complex cases are referred to the Certified Orthotists for a comprehensive assessment.
Prefabricated splints and soft goods - Expert fitting of both lower and upper extremity braces and soft goods including corsets, compression stockings, wrist splints, Air Cast Immobilization Boots, knee sleeves and ankle braces are kept instock for immediate fitting. More complex cases are referred to the Certified Orthotists for a comprehensive assessment.
Custom made foot orthoses - Our Certified Orthotists assess for and provide custom made foot orthoses when appropriate. Each patient is assessed and a detailed history is taken. All patients do require a prescription prior to provision of any Orthotic device.
Custom Orthopaedic Devices - As Certified Orthotists, we assess for, measure, design and fit orthoses for all body parts including but not limited to: knee orthoses, ankle foot orthoses (AFO) spinal orthoses, wrist splints, and elbow orthoses and speciality devices. For eligible patients, The Ministry of Health Assistive Devices Program may provide partial funding when the device is provided by a Certified Orthotist.
Mobility and Gait Consultation - Our Certified Orthotists are trained to assess gait and mobility. Information regarding Orthotic intervention and treatment plans are outlined and reviewed in a report. Recommendations for appropriate interventions including bracing, footwear and other health care professional involvement are made.
